Bring your old HSA here

Have an HSA sitting at another bank? You can move it into your HSA. You don’t send us anything — your current custodian moves the money, and they need a signed instruction from you to do it. We’ll fill that form out for you.

1 We fill out your form Answer five questions and we generate the completed transfer request, with our account details already in place. Takes two minutes

2 We counter-sign it The last section has to be signed by us as the receiving custodian. We email the signed copy back to you. Within one business day

3 You sign and send it Sign it and send it to your current custodian — we give you their address, fax or upload page. Your part

4 They send the money Your old custodian sends a check to our custodian bank. We post it to your account and email you. Usually 2–6 weeks

The account you’re moving

This is the HSA you already have somewhere else — the one you want brought over here.

Who holds it now? Tell us who holds the HSA you’re moving.

Your account number there On your statement from that bank. Enter your account number at your current custodian.

How much? Tell us how much to request.

Don’t take the check yourself: If your old custodian offers to mail the check to you instead, say no. That turns it into a rollover — one allowed per year, with a 60-day deadline and a tax form. A trustee‑to‑trustee transfer has neither.

Your information

This goes on the form, so it has to match the name on the account you’re moving.

Your employer This tells us which plan your HSA belongs to. Enter the employer whose plan your HSA is under.

First name Enter your first name.

Employer employee ID On your pay stub. Enter your employee ID.

Email We’ll email you a copy of the finished form. Enter an email address, like name@example.com.

We’ll email you the counter-signed copy to sign and send on.
